Issue Description: 
After extended run time in heavy disk access applications AND extended power
down times Quantum (model# VK4550J or VK45J05) 4.2GB, 3 1/2-inch, 7200 RPM
Single-Ended, Ultra/Wide SCSI disk drives may fail to spin up.  These  drives
are at risk of premature failure due to oil contamination which causes
a stiction failure in the HDA.  Only disk drives in the following FRU serial 
number range are affected:

        0005181-97041Dxxxx to 0005181-97440Dxxxx
        
Since release of FCO A0113-2 new information has come to our attention:

	The incidence of failure in the field installed base has been
	significantly less than expected.  As of 3/9/98 only eight sites
	world wide had reported significant failue on a world wide installed
	base of over 100K affected units.
	
	We have learned that short duration power off events apparently do 
	not cause failures.

	Sites reporting high failure have observed failures only after power
	off times of a week or more.
		
	Sites reporting high failure have been high duty cycle, heavy disk
	access applications.
	
	Potential for failure exists in a wider serial number range.

We no longer believe proactive replacement of affected product is appropriate
for all customer situations.

Recommended field actions:

	1)  Customer applications that do not fit the profile of heavy disk 
	    access(*) followed by a long(**) power off cycle.

	    		REPLACE ON FAILURE

	    In these situations the very low probability of failure does not
	    justify the disruption to the customer caused by upgrade.

	2)  Customer applications that do fit the profile of heavy disk 
	    access(*) followed by a long(**) power off cycle.

	    		PROACTIVE UPGRADE AVAILABLE
	    		
	    		
 	3)  Any customer with an SSA with drives in the affected S/N range

	         	SMCC IS OFFERING AN UPGRADE PROGRAM.

	    Customers can upgrade to an aggressively discounted A1000 or 
	    A5000.  Available products include:

	    Contact responsible local sales representative for details.

*Heavy disk access  -All sites reporting failure have been described as
		     heavy disk access.   Example our single internal Sun 
		     site which experienced high failure was a benchmarking
		     center.
		     
		     Unfortunately, due to the low number of sites reporting
		     failure we do not have data to quantify the disk duty
		     cycles vs point of failure.

**Long power off cycle  -All sites reporting high failure experienced failure
			 after power off times of days.  Some sites
			 reported numerous trouble free short duration power
			 cycle events (lasting hours) prior to observing 
	 		 failure. In these cases point of failure was after
	 		 power off events of 7-10 days.

In November 1997 a redesign of the drives actuator bearing eliminated use
of the oil contaminant.  Drives manufactured after this date do not have 
the oil present so are not subject to this failure mode.  The affected 
disk drives may be identified in one of two ways;

   1. ONLINE

      Look for Quantum disk drives with model VK4550J and serial number
      range 97041Dxxxx to 97440Dxxx.  See COMMENTS for other options 

   2. VISUAL
   
      Look for     FRU p/n 540-2937 or 540-2938 and 
                   FRU s/n between 0005181-97041Dxxxx to 0005181-97440Dxxxx
                        
      The S/N can be found on the FRU barcode label attached to the disk drive
      bracket and on the disk drive manufacture label.  An inspection of the 
      manufacturer's label on the drive shows model VK45J05.

Replacement Time Estimate: 
30 minutes (per drive)
Special Considerations: 
IMPORTANT! Do NOT spin down affected disk drives for prolonged periods 
	   until software is backed-up or off-loaded AND replacement disk
	   drives are available.

IMPORTANT! For SPARCstorage Arrays, ensure that the Controller contains 3.12 
	   firmware PRIOR to powering down to install a replacement disk. 
	   Failure to do so could possibly cause another failure when
	   attempting to power up, see FINI0339-1 for further details.
Corrective Action: 
Where replacement is approved replace all Quantum 4.2GB (Vendor model number
VK45J05) disk drives having a serial number between 97041Dxxxx and 97440Dxxx 
with Quantum 4.2GB drives with serial numbers outside the affected range or
Seagate 4.2GB drives as follows;

Replace 540-2937-01 with 540-2937-01
Replace 540-2938-02 with 540-2938-02

Submit request for corrective action approval by sending e-mail to the
auto-responding email address...

with 'FCO A0113-3' in the Subject field.
Billing Type: 
Warranty: Sun will provide parts at no charge under Warranty 
           Service. On-Site Labor Rates are based on how the
           system was initially installed.

 Contract: Sun will provide parts at no charge. On-Site Labor Rates
           are based on the type of service contract.

 Non Contract: Sun will provide parts at no charge. Installation by
               Sun is available based on the On-Site Labor Rates 
               defined in the Price List.

i)   In case of Mandatory FCOs, SunService will attempt to contact all known
     customers to recommend the part upgrade.

ii)  For controlled proactive swap FCOs, SunService mission critical support
     teams will initiate proactive swap efforts for their respective accounts,
     as required.

iii) For Replace upon Failure FCOs, SunService partners will implement the
     necessary corrective actions as and when they are required.
