Issue Description:
Enterprise 450 and Enterprise 250 systems with PCI FC-AL host adapter
interface cards may experience PCI fiber channel loop reset on all loops
to the extent that the system becomes unusable. Only by replacing all
affected PCI host adapter interface cards can the system be brought back
to a stable condition.
Only 41 PCI FC-AL host adapter interface cards are affected. The only method
to determine whether a system may be affected is to visually check the serial
number on the PCI FC-AL host adapter interface cards and compare with the
list below. These shipments are likely to have shipped out of Sun between
February 24th, 1998 and mid-July of 1998. All of the affected boards were
shipped as X-options, early on at initial product release and installed in
customer systems in the field. No spares are likely to be affected.

Root Cause Analysis has determined that during a rework to the PCI host
adapter interface card by the vendor, the LASER diode assembly was thermally
over stressed which could cause marginal operation of the PCI host adapter
interface card.
Corrective action was implemented within Enterprise Services via DPCO# 167
on December 16th, 1999.

Corrective Action:
Upon failure or request replace PCI FC-AL host adapter (375-0040-xx)
that has an serial number listed above with a 375-0040-02 (or above)
that does not have a serial number in the above list.
Billing Type:
Warranty: Sun will provide parts at no charge under Warranty
Service. On-Site Labor Rates are based on how the
system was initially installed.
Contract: Sun will provide parts at no charge. On-Site Labor Rates
are based on the type of service contract.
Non Contract: Sun will provide parts at no charge. Installation by
Sun is available based on the On-Site Labor Rates
defined in the Price List.

i) In case of Mandatory FCOs, Enterprise Services will attempt to contact
all known customers to recommend the part upgrade.
ii) For controlled proactive swap FCOs, Enterprise Services mission critical
support teams will initiate proactive swap efforts for their respective
accounts, as required.
iii) For Replace upon Failure FCOs, Enterprise Services partners will implement
the necessary corrective actions as and when they are required.
