Issue Description:
When boards are inserted into Slot 0 or Slot 1 of a Sun Fire 12K/15K
chassis, the board LEDs may fail to illuminate. This could cause the
Support Engineer to believe that there is a hardware fault and lead to
unnecessary hardware replacement. In most cases, the board is good,
and the problem results from a System Management Software
(SMS) issue.
This issue can occur with the following configurations:
Sun Fire 12K/15K systems running SMS 1.2 with patch 112481-07 (or
higher). Sun Fire 12K/15K systems running SMS 1.3.
An affected configuration can be identified by running these commands:
On the System Controllers:
% pkginfo -l SUNWSMSr | grep VERSION
If the VERSION is 1.2, also check:
% showrev -p |grep 112481
When this issue occurs, the LEDs on the Slot 0 and/or Slot 1 boards
will not be lit following insertion into the expander. In addition, it
will not be possible to power on the Slot 0/Slot 1 board. Also,
'showboards' output reports the board type as "Unknown". For example:
% showboards

Although there have been no known occurrences of this issue in the Field,
in-house testing has encountered this condition in approximately 10% of
board insertions.
Starting with SMS 1.2 patch 112481-07, the Slot 0 and Slot 1 power
manager circuits (which reside on the expander) are programmed in latch
mode. In latch mode, if a power fault exists on a Slot 0 or Slot 1
board, power to that slot is latched off. However, at board debut, the
pre-charge of the board causes the power manager to detect a bogus
fault and latches 48v power off. This effectively cuts off
housekeeping power to the inserted board. Without housekeeping, the
LEDs will not illuminate and FRUID is not available (hence, "Unknown"
status in 'showboards').
A future SMS patch is planned to correct this issue. Once available,
this patch can be applied to the System Controllers as a permanent
fix. In the meantime, please use the workaround provided below.

Corrective Action:
The following recommendation is provided as a guideline for authorized
Sun Services Field Representatives who may encounter the above
mentioned issue.
If LEDs fail to illuminate upon board insertion, remove the board and
reinsert again. Repeat until LEDs do illuminate. Be sure to check for
connector/pin damage between removals/insertions.

Implementation Footnote:
i) In case of MANDATORY FINs, Sun Services will attempt to contact
all affected customers to recommend implementation of the FIN.
ii) For CONTROLLED PROACTIVE FINs, Sun Services mission critical
support teams will recommend implementation of the FIN (to their
respective accounts), at the convenience of the customer.
iii) For REACTIVE FINs, Sun Services will implement the FIN as the
need arises.
